Managed Care and Medical Facilities

When applying for Initial Hospital Privileges and/or Managed Care membership – we attach all supporting documents to the application and mail it to the applicant for review and signature.  We will also supply a stamped, addressed envelope for forwarding to the hospital or managed care plan.

Reappointment Applications for Hospitals and/or Managed Care Plans – includes providing copies of all requested documents and complying with the requirements of each facility.

Government Programs

Recredentialing
